Your Hospice Team

We treat our patients as the unique and special individuals they are. We recognize that patients have individual needs, so we create individual care plans that reflect those needs. Most importantly, we include input from families every step of the way.

We maintain an on-call system to be available and accessible 24 hours a day, seven days a week. Under the direction of a physician – our hospice medical director – our care is provided by a team of specially trained caregivers including:

  • Nurses
  • Social Workers
  • Home Health Aides
  • Chaplains
  • Volunteers
  • Bereavement Coordinators
