The following is an overview of EPOCH's Benefit programs. Please
note that some benefits, coverage, and eligibility requirements
may vary depending on the location.
401(k) Retirement Savings Plan
EPOCH offers a 401(k) Retirement Savings Plan, with a variety of
twelve fund options ranging from low to high risk, to choose from.
EPOCH encourages employees to participate in the 401(k) plan because
it offers employees a way to save money two ways: by saving money
for your future retirement needs and by offering pre-tax deductions
so you lower your taxable income as you participate.
Employees, who are at least 21 years of age, who work 20 hours
or more per week are eligible to participate in EPOCH's 401(k) plan
as early as the first calendar quarter after their hire date. In
addition, EPOCH offers a match of 10% of each dollar the employee
contributes up to the first 10% of an employee's contribution. Participants
contributing 10% of their compensation to the Plan are maximizing
the employer contribution, which amounts to 1% of salary. EPOCH
offers an aggressive vesting schedule, you are 100% vested after
two years of employment.
EPOCH accepts rollovers from other qualified 401(k) plans, so
you can take advantage of the investment variety offered. Employees
are eligible for 401(k) loans up to 50% of your vested balance within
the range of $1,000 minimum and $50,000 maximum.
Medical & Dental
EPOCH offers medical & dental coverage to employees and eligible
dependents on a pre-tax basis. Employees are eligible for insurance
coverage by working 30 hours or more each week and by completing
one month of employment. EPOCH offers comprehensive medical insurance
coverage through a fully insured HMO provider, which covers office
and specialist visits, hospitalization and prescription drug coverage,
to name a few. EPOCH offers dental insurance through a fully insured
Dental PPO provider, offering larger savings by using an in-network
dentist. Dental insurance covers preventative, routine and major
services as well as orthodontia (for children only).
Pre-Tax Spending Accounts
The opportunity to save your hard-earned dollars while managing
your various expenses is made easier through EPOCH's two Pre-TaxSpending
Accounts. team members who regularly work 30 or more hours each
week and have completed one month of employment are eligible to
take advantage of EPOCH's Flexible Spending Accounts.

Healthcare Spending Account |

EPOCH allows for a yearly maximum of $5,000 per employee to
be set aside on a pre-tax basis for you to use to pay various
medical and dental expenses you might incur during the year
for yourself and your eligible dependents. You will be reimbursed
for expenses including but not limited to contacts and eye glasses,
orthopedic shoes, dental fees, and hearing devices from your
Healthcare Spending Account when you submit a receipt for the
qualifying expense incurred.

Dependent Care Spending Account |

A yearly maximum of $5,000 can be set aside on a pre-tax basis
to cover expenses you incur for child and/or elder care. This
may mean significant savings for you over the course of a year
and is a benefit worth comparing to the credit available on
your annual tax return.

Tuition Assistance
EPOCH offers tuition assistance to team members working 20 hours
or more each week who have completed six months of employment. An
annual maximum of $500 is available for courses that are job related
part of a degreed program.
Life and Accidental Death and Dismemberment Insurance
EPOCH will pay the full premium amount for a $10,000 life insurance
policy and an additional $10,000 accidental death and dismemberment
policy for all team members who work 30 or more hours each week
and have completed one month of employment.
EPOCH also offers employees an opportunity to purchase additional
life insurance for themselves or dependents on a voluntary basis.
Employees are eligible by working 30 hours per week and must complete
one month of employment. Deductions are made on an after tax basis,
and the policy is portable, meaning that if you no longer work for
EPOCH you can continue to make contributions on your own to keep
your policy active, it goes where you go.
Short Term Disability
A Short Term Disability program is offered to team members who work
30 or more hours each week and have completed one month of employment.
Note: Short Term coverage may vary depending on State mandated disability
benefits.
Time Off Benefits
EPOCH offers a competitive time off program including vacation,
sick time, and paid fixed and floating holiday benefits.
Referral Program
Team members have bonus earning potential through EPOCH's generous
Employee Referral Program. Eligible team members can earn from $250
to $1,000 for referrals of outside applicants for part-time and
full-time licensed and non-licensed positions which result in employment.
